The deceased employee was identified as 47-year-old Chad Crabtree of Owensboro, Kentucky, by Madison County Coroner Stephen P. Nonn. Nonn identified himself as the project manager for the refinery’s subcontractor Sterett Crane & Rigging.
The crane overturned due to a mechanical issue, according to a press release from Nonn’s office. Erker stated that the emergency response team for the refinery was “working to stabilize the effects” of the crane while also attempting to determine what precipitated its collapse.
She reported that the Occupational Safety and Health Administration was also conducting an investigation at the refinery. The Roxana Police Department is also looking into it, according to Nonn.
